Dear Sirs,

we would like to kindly inform you about the change in the legal form of our Company, from a general partnership (spółka jawna) to a limited liability company (spółka z ograniczoną odpowiedzialnością), which took place on 2 January 2026. Since that day, the Company has been operating under the name:
ICB Pharma spółka z ograniczoną odpowiedzialnością with its registered office in Jaworzno
(abbreviated name: ICB Pharma sp. z o.o.).

As a result of the transformation of the Company’s legal form, the Company remains the same legal entity and continues its operations without change. All previously concluded contracts and their terms remain valid and will be performed on the same conditions.

At the same time, we would like to inform you that, as a result of the transformation, the Company’s NIP and REGON numbers remain unchanged. However, the transformed Company has been assigned
a new KRS number: 0001208841. The address of the Company’s registered office has been changed to
ul. S. Lema 10, 43-603 Jaworzno.

Accordingly, with effect from 2 January 2026, all invoices and accounting, commercial, and legal documents should include the following details of the transformed Company:
